He will then discuss those treatment options with you, answer any questions you might have, and design a treatment plan that is best for your case. Dr. Amalfitano makes sure to stay informed of the most recent innovations in periodontics in order to help you save your teeth and avoid unnecessary surgeries.

- Your completed patient registration form
- Your completed medical history form
- A list of medications you are taking
- Your dental insurance information, if applicable

- Patients under 18 will need to bring a parent or guardian to their initial visit.
